Insurance and Billing

We want your experience with Ophthalmology Consultants to be as pleasant as possible and are happy to assist you with your financial arrangements.

If you belong to an insurance plan in which we participate, please bring your identification card and any co-payment with you to your visit. If your insurance plan requires a referral authorization, please contact your insurance company prior to your visit. Please do not hesitate to call us if you have any questions.

Insurance Plans We Accept

Most “vision” plans have benefits that are separate and different from your medical coverage. Please check with your plan to confirm benefits if you are scheduled for a routine eye examination. We are happy to assist you if you need any help in determining your benefits through your carrier.

Below is a partial list of the insurance plans that we currently accept:

If you are on a plan that is not listed here please contact us to see if we participate with your plan. If not we can still see you, but your visit would be considered “out of network” by your insurance carrier.

Please keep in mind that you are ultimately responsible for payment. Any fees not covered by your insurance are due and payable on the day of service, unless prior arrangements have been made. In addition to cash and personal checks, we also accept MasterCard, VISA, American Express, and Discover. We can also arrange for interest free financing through Care Credit.
